CIS Biotech is pleased to offer Gold Dot NR2 Antibody testing in house for those who would like to have it done. CIS Biotech, Inc has a CLIA certificate and Georgia State License for Gold Dot NR2 Antibody test. We can perform the testing at our facility for in and out of state patients if they are referred by authorized physician and also if your state does not have any additional certification requirements for out of state sample shipment and analysis.

Then have your primary care physian fill out his or her information and sign it. You may either fax the form or bring it with you to our facility after an appointment has been made.

There is a small fee associated with the test and CIS Biotech can only accept cash or check at this time. All samples that are going to be shipped to CIS Biotech will need to be paid by the shipper.
