CIS Biotech, Inc., has a CLIA certificate (received on February 26, 2009) and Georgia State License for Gold Dot NR2 Peptide test and Gold Dot NR2 Antibody test. CIS Biotech can perform the testing at its facility for in-state and out-of-state patients if they are referred by an authorized physician (the required paperwork is provided upon request) and if the state does not have any additional certification requirements for out-of-state sample shipment and analysis.
